We at First Responders First, embrace the possibilities that current technology offers, which is why we are now proud to offer our services to include our online Intensive Outpatient level of care treatment program, which is exclusively for First Responders.

Confidential Mental Health Assessment

If you answer “yes” to any of the questions provided, it is highly recommended that you contact the staff at First Responders or another qualified healthcare provider. If you would prefer to be contacted by the staff at First Responders, please leave your contact information in the sections provided at the end of the assessment. Please note that by leaving your information, you consent to allow First Responders to use this information to contact you. Any information provided by you will remain confidential. If you decide not to leave your information, the staff at First Responders will be unable to contact you.

This online Behavioral Health and Substance Abuse assessments take approximately five minutes and will provide general feedback once it has been completed. Please note that this assessment is not a formal diagnostic tool and should not be interpreted as such. This assessment is free of charge and can be taken anonymously, if you so choose.

Do you  find yourself sad, anxious, irritable, or worried most days of the week for  long periods of time?
Do you  have trouble falling asleep or staying asleep?
Do you  feel fatigued or lethargic most of the time, no matter how much sleep you  get?
Do you  ever feel like you are being watched, fearful that someone is constantly out  to get you?
Do you  ever feel a sensation of deep euphoria for no apparent reason, almost as if  you could conquer anything?
Do you  ever compulsively engage in behaviors that you later regret or could  compromise your safety (e.g. gambling, over-spending, shoplifting, or risky  sexual behavior)
Do you  ever feel unable to relax if things aren’t exactly symmetrical, perhaps  engaging in habitual counting or reordering of objects?
Have you  used one substance in order to intensify the high from another substance?
Do you  ever feel unable to leave your home, even when you have work, school, or  social responsibilities?
Do you  ever restrict your food intake or overeat to the point of sickness?
Do you  struggle to control your temper, often feeling high levels of rage?
Do you  ever have thoughts of harming others, and have you ever made a plan to do so?
Have you  had thoughts of harming yourself, or have you ever made an attempt to take  your own life?
Do you  continually experience racing, intrusive thoughts that you can’t seem to  quiet?
Do you  regularly use substances like alcohol or illicit drugs, often feeling unable  to function without them?
